Our services are comprehensive, offered in a way that allows each family to gather the information and support that best suit their needs.


Assessment
of client's capabilities and limitations in all areas of independent living. This includes tasks of daily personal care, homemaking, community activities, and social connections. Additionally, home environment safety is studied to minimize the risk of falls or accidents.

Care Planning pulls together the information from the Assessment into a workable, step- by-step action plan. The plan, also, incorporates input from family members and other caregivers, the client, and relevant professionals.

Service Coordination and Monitoring includes locating, screening, scheduling and monitoring all caregivers, contractors, and other service providers; coordinating medical and rehabilitation appointments, transportation, social programming, and follow-up services; and ongoing communication with concerned family members.

Advocacy for patient's rights and services within the healthcare delivery system, entitlement programs, or within day or residential programs

Consultation/Evaluation can be a one-time meeting with family members or other concerned parties on urgent issues, or meetings scheduled only on an as-needed basis.

Caregiver Coaching provides family caregivers with ongoing professional assistance or guidance to solve difficult problems or adapt to the changing needs of their loved one.

A Virtual Family Support Group meets on-line or in a conference call setting to enable information sharing with others in similar circumstances.

Details of services

Assessment

An objective format looks at cognitive and physical strengths and weaknesses to identify areas requiring assistance to continue to live independently. Assessment includes:

  • Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, eating, managing medications
  • Homemaking, such as cooking, laundry, light housekeeping, using telephones, managing finances
  • Community activities, such as transportation, mail, banking, grocery shopping
  • Social connections, including attending religious services, and meeting with friends

    Assessments also include a review of the important documents that should be in place, such as assignment of a Healthcare Proxy and a Power of Attorney, availability of an emergency plan, and an estate plan.

Care Planning

Initially created as a "work-in-progress," the Plan of Care uses information from the Assessment to develop a step-by-step action plan. The proposed plan also factors in family time and resources, informal caregivers, the client's daily routines, and physician recommendations. Options for different types of care are offered, with their related expenses. The plan is reviewed with the client, the family, and other relevant persons for further decisions or changes. Ultimately, the plan comprehensively meets the client's, and the family's, needs.

Plans typically include:

  • Short term plans - to resolve immediate problems and address needs
  • Long terms plans - to address ongoing need for care and assistance, and/or preparation for transitions, if necessary
  • Referrals - to review geriatric medical, legal and financial issues with the appropriate specialists

The family and client can:

  • choose to implement the plan themselves
  • decide to share the responsibilities with the Care Manager
  • have the Care Managers oversee the plan in its entirety with regular updates to the family.

Plans are reviewed on a regular basis and are adjusted to reflect any changes in the client's needs.

Coordination/Monitoring of Services

Includes identifying, interviewing, selecting, scheduling and monitoring service delivery sources, such as in-home care, specialized equipment, service contractors, rehabilitation specialists, transportation services, and a myriad of others.

The Geriatric Care Manager maintains an ongoing relationship with the client by continually monitoring all provided services. This generally includes visits on a regular basis, scheduling and attending important appointments, following up on recommendations, untangling complications, and being there as the local person for everyday problem solving or reassurance.

At Independent Living Solutions, Inc. we communicate regularly with family members, letting them know that things are going well, or alerting them to possible problems before they escalate into crises.

Advocacy

For many people, successful transition to an Assisted Living residence or Nursing Home requires a lot of support. These transitions are major life changes, often creating significant stress on normally adequate coping mechanisms. Even the best facilities' promised services are not always delivered in ways or times that meet the new resident's needs, and schedules of activities or other offerings are not always obvious: The new resident may be too overwhelmed to know how to ask for help. Independent Living Solutions arranges frequent visits, monitors the delivery of services, and negotiates with care providers to ensure that the client is receiving the maximum benefit.

In the community, our Geriatric Care Managers function as advocates for their clients when dealing with any bureaucracy -- applying for community programs or housing, dealing with insurance companies, or arranging necessary medical care. This includes doing the leg work to make sure paper work is complete, tests or consultations are scheduled, reports are sent to (and received by) the right office, bills received are accurate, and appropriate action is taken.

Consultation/Evaluation

Sometimes families know something needs to change, but are uncertain of their options. A Geriatric Care Manager from Independent Living Solutions can consult with the concerned parties to discuss the problem, identify available options, and generate process guidelines. If the problem is not well defined, ILS will meet the client in their environment before identifying the most appropriate options.

Caregiver Coaching

For families managing the care of their aging parent themselves, Independent Living Solutions offers the opportunity to receive support and advice from a Geriatric Care Manager. The benefits include meetings at their convenience (in person, by phone, on-line, during the day or in the evening), and having access to the professional insight needed to address on-going concerns, resolve immediate problems, adapt to changing functional levels, and understand the options available at each turn.

Virtual Caregiver/Family Support Group

Caregivers do everything possible to help their loved ones, many times at a high cost to their own well-being. It's not unusual for them to feel isolated, to feel that no one can really understand the demands they work under every day. Independent Living Solutions offers a support group to caregivers who cannot attend a traditional support group at a hospital or clinic, but would welcome the opportunity to exchange thoughts, feelings, and ideas with others facing similar daily challenges via an on-line or telephone-based discussion.
